We are seeking a dedicated and compassionate individual to join our team as an Assessment and Reconnection Worker. As part of our Pan London Rough Sleeping Team, you will play a vital role in supporting people at risk of rough sleeping and making a positive impact on their lives.

Key Responsibilities
  • Provide personalized case management services to clients, turning assessment recommendations into practical action plans that fully involve the client.
  • Build and maintain effective relationships to support clients throughout their recovery journey.
  • Develop a knowledge of relevant legislation, including welfare benefit and housing legislation.
  • Work on a weekly rota, including early, mid, and late shifts, with some evening and weekend work required.
