About the Role

We are seeking a highly skilled and experienced Nurse Practitioner to join our team at Central and North West London NHS Foundation Trust. As a key member of our primary care team, you will play a vital role in delivering high-quality healthcare services to patients within our trust.

Key Responsibilities

  • Triage patient needs and assess, plan, and implement care for patients within each care/treatment pathway to promote wellbeing.
  • Facilitate the use of evidence-based practice to ensure that clinical care is both effective and appropriate.
  • Carry out general nursing duties and maintain contemporaneous records.
  • Support and make a professional contribution to the development of integrated healthcare services.
  • Act as a preceptor/mentor in accordance with experience.

Requirements

  • First Level/Registered Nurse qualification.
  • Evidence of personal development in previous employment.
  • Ability to demonstrate a working knowledge of health promotion and communicate effectively with patients and colleagues.
  • Good working knowledge of IT and emails.
  • Display an understanding of the NHS agenda with regards to offender health and an appreciation of the roles of the members of a multi-disciplinary team.
