Key Responsibilities
- Provide evidence-based care and support to service users, promoting their physical and emotional well-being.
- Work collaboratively with colleagues to deliver person-centered care, prioritizing the needs of service users.
- Participate in multidisciplinary teams to develop and implement care plans, ensuring seamless transitions and continuity of care.
- Contribute to a culture of safety and quality, identifying and reporting any concerns or incidents.
- Engage with service users, their families, and carers to promote open communication and build strong relationships.
Requirements
- NMC registration and a relevant nursing qualification.
- Strong team-working skills and a flexible approach to meet the needs of service users.
