We are seeking a highly skilled and experienced Registered Nurse to join our team as a Complex Community Care Specialist. As a key member of our community care team, you will be responsible for assessing and reviewing individual care packages, providing practical training to care workers, and clinical supervisions and instructions using your clinical expertise and guidance.
Key Responsibilities:
- Assess and review individual care packages to ensure high-quality care is delivered
- Provide practical training to care workers to enhance their skills and knowledge
- Conduct clinical supervisions and instructions to ensure best practice is followed
- Work collaboratively with healthcare professionals, including ICBs, Case Management, Social Services, and private and NHS hospitals
- Participate in our paid phone support On Call Rota on a rotational basis
Requirements:
- Registered Nurse with experience in ICU/HDU or complex care
- Self-motivated, enthusiastic, caring, empathetic, and compassionate
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
