Role Overview:

An exciting opportunity exists for a qualified and driven Registered Nurse (RNLD/RMN/RGN) to take on the role of Nurse Assessor in the Northeast of England. This position is exclusively Non-Term Time.

This role is community-focused, providing a platform to enhance your assessment and decision-making abilities in a supportive yet challenging environment.

As a Nurse Assessor, you will:

  • Demonstrate motivation and a desire to acquire new skills.

  • Possess strong verbal and written communication skills, with meticulous attention to detail.
  • Showcase excellent interpersonal skills, particularly in managing challenging situations and individuals.

  • Prescribe care tailored to individual health requirements.
  • Uphold a strong commitment to quality and patient/career experience.
  • Exhibit effective time management skills and thrive in a fast-paced environment.
  • Have proficiency in Microsoft Office and intermediate typing abilities.
  • Be prepared for last-minute changes.
  • Travel daily across the region and occasionally nationwide.

Main Responsibilities:

  • Collaborate within the commissioning team and engage with a wider multidisciplinary team (MDT) both internally and externally.
  • Conduct comprehensive care needs assessments.
  • Facilitate multidisciplinary assessments and resident reviews in accordance with established policies and procedures, working closely with Home Managers.
  • Manage cases as directed by the Head of Commissioning.
  • Act as Duty Clinician when necessary.
  • Maintain up-to-date data systems in line with operational policies.
  • Comply with the Data Protection Act and confidentiality standards.
  • Engage in ongoing professional development and clinical supervision as per NMC guidelines.
  • Ensure compliance with all mandatory training requirements.
  • Maintain professional registration through revalidation.
  • Provide coverage for other Nurse Assessors as needed.
  • Ensure professional and appropriate communication at all times.
  • Support the Continuing Healthcare department in achieving departmental goals.
  • Foster constructive relationships with a variety of internal and external stakeholders.
  • Communicate relevant information, risks, and issues, including briefings and reports.
  • Work closely with all care providers involved in service delivery.
  • Coordinate and participate in relevant meetings, reporting attendance and providing information and support as requested.
  • Ensure accurate and legible documentation of information.
  • Assist and support individuals and their representatives.
  • Enhance current management information systems to improve decision-making processes.
  • Conduct research and development as directed.
  • Carry out reviews and other delegated duties as required.
