Community Hospice & Palliative Care is seeking a skilled Hospice APRN to join our team. As a key member of our interdisciplinary team, you will work collaboratively with physicians and other healthcare professionals to deliver high-quality patient care and related health services to patients and families in various settings.
Responsibilities:- Conduct thorough medical examinations and evaluations, diagnose, and develop treatment plans for patients and families.
- Assist with the diagnosis, treatment, and management of acute and chronic health conditions, ensuring comprehensive care for patients.
- Order, perform, and interpret laboratory and radiology tests within the scope of your professional practice.
- Write orders for informed consents, diagnostic procedures, and treatments, and prescribe medications as needed.
- Consult with physicians and other healthcare professionals as required, ensuring seamless communication and collaboration.
- Maintain accurate and detailed patient records, charts, and documents to support sound medical practice and reimbursement of services provided.
- Execute physician orders in accordance with legal, ethical, and organizational standards.
- Comply with applicable laws and regulations, ensuring timely reimbursement for services rendered.
- Ensure patients and families are informed about CPR and DNR procedures.
- Recommend medications as required to treat specific patient populations.
- Adhere to confidentiality standards regarding patients, families, staff, and the organization.
- Provide educational sessions to advance hospice care among internal staff, medical professionals, and the community.
- Participate in on-call coverage and assist in establishing schedules.
- Participate in regular interdisciplinary team meetings to review active cases.
- Adhere to internal controls established for the department.
- Coordinate Inpatient APRN work assignments.
- Communicate potential assignment changes based on daily census.
- Review and coordinate Inpatient APRN PTO requests to ensure staff coverage meets current patient needs and census.
- Prepare group texts to ensure proper communication to all Inpatient APRNs.
- Update and consult with the Chief Medical Officer on APRN staff updates when necessary.
- Act as the point person for Inpatient APRN staff during emergency alert situations.
- Perform other duties as assigned.
- Master's degree in a relevant field.
- Minimum of two years of nursing experience.
- Experience in hospice palliative care is preferred.
- Valid State of Florida Driver's license with an acceptable driving record.
- Advanced Practice Nurse or PA licensed in the State of Florida.
