We are seeking a highly motivated and experienced Housing Support Coordinator to join our Darlington housing support team. As a key member of our team, you will work closely with individuals with complex mental health needs to provide intensive housing support and help them achieve their goals.
Key Responsibilities- Manage a caseload of service users, creating and implementing personalized support plans to meet their housing needs.
- Provide support and guidance to individuals to sustain and maintain their accommodation, as well as maximize their finances.
- Facilitate and promote the progression of individuals to increase their skills and confidence to live independently.
- Collaborate with housing providers, statutory services, and local organizations to achieve the best outcomes for service users.
- Experience of working with vulnerable adults with mental health needs.
- Ability to manage, plan, and prioritize your own workload, delivering high-quality work.
- Understanding of Safeguarding principles.
- Genuine interest in being part of Humankind's mission and vision.
- Strong communication and interpersonal skills.
