We are seeking two experienced Occupational Therapists to join our team at Tripod Partners. As a Reablement Specialist, you will be responsible for conducting face-to-face reviews and assessments for adults of all ages, focusing on reablement, accommodation, equipment, and adaptations.
Key Responsibilities:- Conduct thorough assessments to identify individuals' needs and develop personalized plans for reablement and support.
- Collaborate with clients, families, and healthcare professionals to ensure seamless care coordination.
- Provide expert advice on equipment and adaptations to enhance clients' independence and quality of life.
- Triage cases to determine the most effective use of community resources, reducing the need for statutory services.
- Full qualification as an Occupational Therapist.
- Experience in adult rehabilitation and reablement services.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
