We are seeking two experienced Occupational Therapists to join our team at Tripod Partners. As a Reablement Specialist, you will be responsible for conducting face-to-face reviews and assessments for adults of all ages, focusing on reablement, accommodation, equipment, and adaptations.

Key Responsibilities:
  • Conduct thorough assessments to identify individuals' needs and develop personalized plans for reablement and support.
  • Collaborate with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Provide expert advice on equipment and adaptations to enhance clients' independence and quality of life.

Requirements:
  • Full qualification as an Occupational Therapist.
