About the Role

We are seeking a skilled and compassionate Community Rehab Assistant to join our team. As a key member of our Intermediate Care Team, you will play a vital role in providing rehabilitation and care to individuals in their own homes.

Working closely with our senior team members, you will assist in delivering high-quality care and rehabilitation services that meet the unique needs of each patient. Your expertise and dedication will help us achieve our goal of preventing unnecessary hospital admissions and promoting independence for our clients.

Key Responsibilities

As a Community Rehab Assistant, your primary responsibilities will include:

  • Providing rehabilitation and care services to patients in their own homes, working closely with our therapy team to develop and implement personalized care plans.
  • Assisting with home exercise programs and promoting physical activity to support patient recovery and independence.
  • Maintaining accurate records and reporting any changes in patient condition to our Operational Lead.
  • Providing clerical and administrative support to the team, ensuring seamless communication and coordination of care services.
  • Delivering care in accordance with our organization's policies, procedures, and guidelines, adhering to the highest clinical and quality standards.
