Position Overview
The Patient Pathway Coordinator (PPC) will play a crucial role in supporting the Ophthalmology department, ensuring that patients receive timely management in accordance with established guidelines. This position serves as a vital link between healthcare professionals, administrative staff, and patients to enhance and streamline the overall patient journey.
Key Responsibilities include:
- Overseeing daily management strategies for patients through collaboration with healthcare providers regarding:
- Diagnostic procedures
- Treatment strategies
- Evaluations and assessments
- Continuous care
- Ensuring all patient interactions are accurately documented and entered promptly into the Patient Administration System (PAS).
- Maintaining a precise tracking system to monitor adherence to established treatment timelines for all patients.
- Collaborating with healthcare providers, service managers, and appointment scheduling staff to address any patients not booked within the required timelines, implementing necessary actions to ensure compliance.
- Working alongside the Lead Clinician and Ophthalmology Access Manager to assess service demands and available resources, making adjustments as needed to uphold a compliant service.
- Generating regular reports on patient pathways and compliance tracking for Trust clinicians, managers, and relevant representatives.
The ideal candidate must possess strong prioritization skills and the ability to work independently, often in high-pressure situations.
Primary Duties
- Coordination of individual patient clinical pathways
- Maintenance of a patient tracking system to ensure adherence to treatment protocols and timelines
- Close collaboration with healthcare providers, service managers, ophthalmic nursing staff, administrative personnel, and appointment coordinators to facilitate new patient clinics and assessments within specified timelines
- Ensuring the integrity of the PAS system through thorough reconciliation and validation of patient event data across various platforms
Additional Responsibilities
- Engaging with clinicians and the Ophthalmology Access Manager regarding service demands and resource availability, considering planned absences to maintain service delivery
- Coordinating with Trust support service departments to ensure effective Ophthalmology service delivery, including outpatient services and medical records
- Assisting the Lead Clinician and Ophthalmology Access Manager in developing service information systems, including pathway documentation and patient information
- Collaborating with relevant departments to coordinate service reporting and address patient inquiries, service feedback, and audit requirements
- Supporting the Lead Clinician in planning and booking treatment sessions, aiming for improved productivity
- Providing daily support to the Ophthalmology Access Manager in managing service development projects across various sites
- Organizing meetings as necessary and documenting minutes.
