Position Overview:
We are seeking a dedicated individual to provide essential support services within the community. This role involves working a full-time schedule of 37 hours per week.

Key Responsibilities:
1. Evaluate the requirements of individuals experiencing homelessness and connect them with suitable transition programs.
2. Foster collaborative relationships with partner accommodation providers.
3. Conduct independent visits to clients residing in temporary accommodations across the region.
4. Manage a caseload of 20-25 families for a duration of three months, ensuring their needs are met effectively.
5. Transition families to dedicated Support Workers who will assist them throughout their tenancy, managing a larger caseload of 40 families.

Essential Qualifications:
- A valid driving license and access to a vehicle for work-related travel, including appropriate business insurance.
- An up-to-date enhanced DBS check for both adults and children.
- Previous experience in the field of homelessness is preferred.
