Position Overview

Hywel Dda University Health Board is seeking a dedicated and enthusiastic individual to join our Medical Therapy Team as a Rehabilitation Support Practitioner. This role is integral to our multi-disciplinary approach, collaborating with various healthcare professionals including support staff, physiotherapists, and occupational therapists.

Role Responsibilities

The successful candidate will play a vital role in delivering rehabilitation services to patients with diverse medical conditions. You will work closely with qualified therapists and independently to help patients reach their rehabilitation objectives and facilitate safe and timely discharges.

Your responsibilities will include:

  • Implementing rehabilitation programs for patients on the Medical Wards.
  • Delivering exercise regimens and assisting with gait re-education.
  • Supporting patients in regaining independence in daily activities, such as personal hygiene and dressing.
  • Collaborating with a team of physiotherapists and occupational therapists, as well as the broader multidisciplinary team.

Essential Qualifications and Experience

To be considered for this role, candidates should possess:

  • Clinical experience within a therapy service.

  • A Level 3 qualification relevant to the role or equivalent experience.
  • Strong IT skills, particularly in Microsoft Office applications.
  • Numeracy and literacy skills at a minimum of Level 2.

Desirable Attributes

While not essential, the following qualifications and experiences would be advantageous:

  • Experience in various team settings and group environments.
  • Knowledge of clinical governance and relevant outcome measures.
  • Understanding of rehabilitation interventions across multiple therapy disciplines.
