About the Role

We are seeking a highly skilled and motivated Senior Physiotherapist to join our Rapid Access Service team. As a Senior Physiotherapist, you will be responsible for providing high-quality, evidence-based interventions to patients in their own homes, as well as leading the therapy pathway for non-injured fallers.

Key Responsibilities
  • Provide complex assessments and interventions to patients in their own homes
  • Lead the therapy pathway for non-injured fallers
  • Work collaboratively with the multidisciplinary team to ensure seamless care
  • Develop and implement individualized care plans to meet the needs of patients
  • Contribute to the development of the Rapid Access Service and its strategies
Requirements
  • HCPC registration as a Physiotherapist
  • Experience of working in a community setting
  • Experience of working with complex and frail patients
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
