About the Role

We are seeking an experienced and enthusiastic Community Health Nurse to join our team at Harrogate and District NHS Foundation Trust. As a Community Health Nurse, you will play a vital role in delivering high-quality care to children and families in the Wakefield district.

Main Responsibilities
  • Contribute to the delivery of the Healthy Child Programme by undertaking targeted interventions and health promotion as directed by the Specialist Community Public Health Nurse.
  • Support the completion of comprehensive health needs assessments of clients, families, and community, and facilitate the mobilization of appropriate resources to meet identified needs.
  • Ensure inter-agency and professional partnership working between services, statutory, and voluntary organizations to facilitate integrated approaches in meeting the needs of children.
  • Develop and maintain effective relationships with parents, carers, children, and partner agencies to promote the physical, emotional, and social development of children.
Requirements
  • Registered Nurse with experience of working with children and families.
  • Experience of working as part of a multidisciplinary team.
  • Good communication skills and ability to travel independently.
