About the Role:
Hatched Recruitment Group is seeking a dedicated and experienced Qualified Social Worker to join our dynamic and supportive team in Adult Services. As a Qualified Social Worker, you will have the opportunity to make a real difference in the lives of young people with additional needs and their families.
Key Responsibilities:
- Conduct comprehensive assessments and develop person-centered support plans for young people and their carers, ensuring timely and effective identification of eligible social care needs.
- Collaborate with multidisciplinary teams, including children's services, education, healthcare professionals, and community partners to provide a holistic approach to meeting the needs of young people.
- Monitor, review, and maintain up-to-date support plans, ensuring they meet the changing needs of young people and reflect good value for money.
- Safeguard and promote the welfare of young people, adhering to relevant legislation and policies, and identifying and assessing levels of risk and need in complex situations.
- Deliver protective and/or supportive services for individuals at risk, including work required by the Court of Protection.
