Specialist Practitioner District Nurse & Community Nurse Practitioner

We are seeking highly skilled and dedicated individuals to fill the role of Specialist Practitioner District Nurses and Community Nurse Practitioners across Edinburgh and East Lothian.

About the Role

As a Specialist Practitioner District Nurse, you will be responsible for providing high-quality nursing care to patients in their homes and communities. You will work closely with other healthcare professionals to deliver comprehensive care and support to patients with complex needs.

As a Community Nurse Practitioner, you will be responsible for providing advanced nursing care to patients in their homes and communities. You will work autonomously to assess, diagnose, and treat patients with a range of health conditions.

Key Responsibilities
  • Provide high-quality nursing care to patients in their homes and communities
  • Work closely with other healthcare professionals to deliver comprehensive care and support to patients with complex needs
  • Assess, diagnose, and treat patients with a range of health conditions
  • Develop and implement individualized care plans to meet the unique needs of patients
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
Requirements

To be successful in this role, you will need to have:

  • Registered Nurse qualification and current registration with the NMC
  • Postgraduate qualification in District Nursing or Community Nursing
  • Experience of working in a District Nursing or Community Nursing role
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
