Job Opportunity

We are seeking a dedicated and compassionate individual to join our team as a Support Worker in our Chorley ABI Service. This role offers a unique opportunity to make a positive impact on the life of an individual with complex needs, providing tailored support to enable them to live a fulfilling and valued life.

The successful candidate will be responsible for delivering high-quality support services, promoting independence, and empowering the individual to achieve their personal goals. This will involve working collaboratively with the individual, their family, and other stakeholders to develop and implement personalized support plans.

Key Responsibilities:

  • Deliver high-quality support services to the individual, promoting independence and empowerment
  • Develop and implement personalized support plans in collaboration with the individual and other stakeholders
  • Work collaboratively with the individual's family and other professionals to ensure seamless support
  • Contribute to the development and implementation of service policies and procedures
  • Participate in ongoing training and professional development to enhance skills and knowledge
