About the Role:

Care First UK Recruitment Solutions is seeking a skilled and experienced Trusted Assessor to join our team. As a Trusted Assessor, you will play a pivotal role in reducing delayed transfers of care from acute settings.

Main Responsibilities:

  • Ensure patient assessments are completed within a 2 to 4 hour time window and their onward destination arranged and agreed between all relevant parties.
  • Negotiate between care home managers and service users to facilitate discharge effectively to the agreed destination within hours.
  • Partner with health and social care colleagues to maintain effective patient flow through the health and social care system.
  • Develop and maintain relationships with hospital discharge teams and care setting management teams to raise the profile of the Trusted Assessor role.
  • Develop the Trusted Assessor pathway and processes, and identify ways to improve current culture, methodology, and processes.
  • Communicate with social care and commissioning colleagues to ensure agreed block capacity is utilized in an appropriate and timely fashion.
  • Develop performance reporting, including completed assessments, transfers to care homes, and level of care home engagement.
