About the Role
This is an exciting opportunity to join Dorset Clinical Commissioning Group as a Physical Health Support Worker - Specialist Role. As a key member of our team, you will play a vital role in providing high-quality care and support to our patients.
Key Responsibilities
- Assess, plan, implement, and evaluate programmes of care for patients with complex physical health needs.
- Work collaboratively with multidisciplinary teams to provide comprehensive care and support.
- Provide clinical supervision and mentoring to junior staff to ensure high standards of care.
- Contribute to the development and delivery of training programmes to enhance the skills and knowledge of staff.
- Use electronic patient/service user record systems to record and analyze data, producing reports as required.
Requirements
- Registered practitioner (MH nurse, OT, or social worker) to degree/diploma level, supplemented by post-registration diploma-level specialist training and/or short courses.
- Membership of the relevant Professional Body.
- Evidence of recent professional development in an up-to-date portfolio.
- Ability to travel using own vehicle on Trust business, subject to the provisions of the Equality Act.
- Demonstrable skills in written and spoken English to a standard enabling the post holder to carry out the full range of duties and responsibilities effectively.
