Job Title: Functional Nurse/Physiotherapist Assessor

We are seeking a highly skilled and experienced nurse or physiotherapist to join our team as a Functional Nurse/Physiotherapist Assessor. As a key member of our assessment team, you will be responsible for conducting thorough and accurate assessments of clients, providing expert advice and guidance, and contributing to the development of effective care plans.

Key Responsibilities:
  • Conduct comprehensive assessments of clients, including physical and functional evaluations, to identify areas of need and develop effective care plans.
  • Provide expert advice and guidance to clients, families, and healthcare professionals on matters related to functional assessment and care planning.
  • Collaborate with multidisciplinary teams to develop and implement care plans that meet the unique needs of each client.
  • Contribute to the development and maintenance of assessment tools and protocols to ensure consistency and accuracy in assessment practices.
  • Participate in quality improvement initiatives to enhance the quality of care and services provided.
Requirements:
  • At least one year of post-registration experience as a nurse, occupational therapist, physiotherapist, or paramedic.
  • Strong observational and communication skills, with the ability to listen, empathize, and investigate thoroughly.
  • Ability to work efficiently, meet deadlines, and produce precise reports.
  • Proficiency in IT, Microsoft Office, and remote systems.
  • Valid driving license or reliable transportation to reach the nearest assessment centre.
