Job Title: Community Health Lead

We are seeking an experienced and skilled Community Health Lead to join our team at Bradford District Care NHS Foundation Trust. As a Community Health Lead, you will be responsible for providing leadership and management to a team of clinical practitioners, working together to deliver high-quality adult physical health services in the community.

The successful candidate will have a district nurse qualification or equivalent professional qualification, as well as current or recent operational management experience. You will be responsible for managing and developing staff, and will have experience of leading a service reconfiguration and improving professional practice.

As a Community Health Lead, you will work closely with Operational Managers and Clinical Team leaders to deliver on our transformational plans to implement the NHS plan. You will be responsible for providing proactive safe care to our local population, and will work alongside other key members of the primary health care team, and other health and social care professionals.
