About the Role

We are seeking an experienced Registered Nurse to join our team as a Community Nurse. As a Community Nurse, you will provide high-quality nursing care to patients in their own homes, working closely with a multi-disciplinary team to deliver excellent patient outcomes.

Key Responsibilities
  • Assess the healthcare needs of patients and families, monitoring the quality of care they receive.
  • Be professionally accountable for the delivery of care, utilising technology to update records as you go.
  • Work flexibly as part of a team, prioritising work based on patient need.
  • Provide acute nursing assessment and interventions in patients' own homes, ensuring patients are referred onwards as appropriate to community services.
