Your Role at Prestige:
We are seeking a proactive Care Coordination Specialist with robust planning and scheduling expertise, complemented by a flexible and positive mindset, to join our dynamic team.
You will collaborate closely with your Branch Manager to ensure that essential business and compliance objectives are achieved.
As we focus on sustainable growth, this position offers ample opportunities to contribute to the organization's success.
The primary responsibility of the Care Coordination Specialist is to advocate for Prestige Nursing & Care while ensuring that the deployment of nursing and care resources is optimized and aligned with client needs, adhering to established quality, compliance, and safety standards.
Your success in this role will be evaluated based on:
- Efficient shift scheduling and proactive planning to guarantee all clients are matched with appropriate frontline staff.
- Identifying opportunities for business expansion.
- Building strong relationships with frontline colleagues.
- Meeting business performance indicators.
Key Responsibilities:
- Ensure daily coverage for all client visits.
- Organize quality assessments for Field Care Supervisors & Field Care Managers.
- Collaborate with Branch Nurses to support the evaluation, training, and competencies of complex care packages.
- Deliver outstanding customer service to both internal and external stakeholders.
- Communicate effectively with various healthcare professionals, social workers, and case managers.
- Maintain compliance with all operational systems and procedures.
- Take ownership of your area and work collaboratively with other care coordinators to ensure care coverage.
- Keep frontline colleagues informed about changes in client needs.
- Foster strong relationships with existing clients and colleagues.
- Identify new business opportunities to support growth alongside the Branch Manager.
- Collaborate with multidisciplinary health teams to maximize client care opportunities.
- Ensure the effective placement of frontline staff to meet client requirements.
- Prepare reports as needed.
- Participate in the on-call rota and provide weekend cover as required.
General Responsibilities:
- Assist with daily team responsibilities as needed.
- Champion internal customer service.
- Undertake any other reasonable tasks as directed by your manager.
