About the Role

We are seeking a highly motivated and skilled Mental Health Practitioner/Care Coordinator to join our team at Nottinghamshire Healthcare NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to our patients and their families.

Key Responsibilities
  • Manage a caseload of patients under the Care Programme Approach
  • Conduct initial assessments and one-to-one interventions with service users
  • Develop and implement treatment plans to meet the needs of service users
  • Establish and maintain effective communication networks with clients, carers, and other healthcare professionals
  • Use interpersonal skills to diffuse potentially emotive situations
Requirements
  • Professional qualification in Mental Health to degree level or equivalent
  • Current registration with the NMC
  • Practice-based educator qualification or equivalent
  • Ability to work as part of a multidisciplinary team
  • Good assessment, intervention, and evaluation skills
  • IT skills
  • Ability to prioritize work
  • Interpersonal skills
  • Clinical supervision skills
