Job Summary

Braintree Primary Care Network is seeking a highly skilled and motivated Clinical Pharmacist to join our team on a full or part-time basis. The successful candidate will be responsible for providing clinical expertise and leadership in the use of medicines and medicine information to patients and healthcare professionals within our network.

Main Responsibilities
  • Provide clinical assessments and treatments to patients, utilizing expert knowledge of medicines and disease areas.
  • Offer specialist expertise in the use of medicines and medicine information, including side effects and dosage.
  • Conduct pharmaceutical medication reviews and provide health education advice to patients through face-to-face, telephone, or notes-based reviews.
  • Lead on person-centered medicines optimization and quality improvement initiatives.
  • Contribute to public health campaigns, including flu vaccinations, childhood immunization, and adult immunization programs.
  • Act as a key contact for medication-related queries.
  • Support improvements in clinical care through practice-based audits and implementing changes.
  • Run clinics focused on medicines as the primary intervention, including anticoagulant monitoring clinics.
  • Conduct care planning, including mental health, admission avoidance schemes, and end-of-life patient care.
  • Review daily pathology results and take appropriate action.
  • Undertake minor ailments triage and direct patients based on clinical need.
  • Capture relevant patient health information at all consultations, contributing to the quality outcomes framework.
  • Monitor patients with complex, long-term conditions, ensuring medication is optimized, titrated, monitored, and complied with.
  • Provide clinical advice and expertise on medication.
