Clinical Pharmacist Position Overview:

Compensation: £51,488- £57,800 (commensurate with experience)

Work Schedule: 37.5 hours per week

Reporting To: Deputy Head of PCN Pharmacy/ Head of PCN Pharmacy

ORGANIZATION BACKGROUND:

Healthcare Central London operates as a GP Federation serving the Central London borough area. This organization supports a network of 33 General Practices and 4 Primary Care Networks (PCNs), with a mission to be acknowledged as a premier GP provider network, managed by healthcare professionals for the benefit of the local community.

ROLE DESCRIPTION:

  • Function as a vital member of a multi-disciplinary team in a patient-centric capacity
  • Oversee chronic disease management and conduct clinical medication assessments
  • Offer primary assistance to general practice personnel regarding prescription and medication inquiries
  • Facilitate collaboration with community and hospital pharmacy to enhance patient outcomes

PRIMARY DUTIES AND RESPONSIBILITIES:

  • Execute structured medication evaluations
  • Perform clinical medication assessments with patients
  • Deliver pharmaceutical support, including telephonic consultations
  • Detect and correct medication discrepancies to avert harm
  • Engage in service enhancement and quality improvement initiatives

KEY WORKING RELATIONSHIPS:

Establish and maintain connections with both internal and external stakeholders, including General Practice and North West London ICB.

Qualifications:

Licensure: General Pharmaceutical Council (GPhC)

Education / Training / Credentials: Master's degree in pharmacy, Completion of 18-month CPPE pathway, Independent prescriber status

Expertise and Experience: Previous experience in GP/clinical pharmacy, Specialized knowledge in chronic disease management

Competencies: Ability to plan, manage, and oversee pharmaceutical care programs, Accountable for professional knowledge, Ethical decision-making capabilities
