We are seeking an experienced Social Worker to join our Transfer of Care Team in partnership with Southwark. The successful candidate will have a strong background in adult services and be able to work effectively with a mixed caseload.
Key Responsibilities- Cover duty and work on INT with a mixed caseload, completing assessments
- Support planning and reviews, including mental capacity assessments and best interest decisions
- Attend active case management MDTs and CHC meetings
- Support individuals with complex needs
- Competitive hourly rate of up to £32
- Full-time contract available
