Key Responsibilities- Manage the care of patients with long-term conditions, including diabetes management and chronic disease management.
- Provide a range of nursing services, including venepuncture, ECG measurement, blood pressure measurement, urinalysis, immunisations, and wound care.
- Support effective treatment and prescribing by working with patients to support adherence to prescribed treatments and creating care plans.
- Educate and support patients as needed, including adopting healthy lifestyles and appropriate self-care.
- Provide a sexual health service where appropriate.
- Assess the needs of patients and provide appropriate care/treatment in conjunction with other healthcare professionals.
- Refer to colleagues or other agencies as necessary.
- Prioritise health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations.
- Minimum of 2 years post-registration experience in Primary Care General Practice.
- Experience of dealing with patients with undifferentiated and undiagnosed conditions, including chronic diabetes.
- Accredited training in diabetes management.
- Registered General Nurse (currently registered with the Nursing & Midwifery Council) and current NMC registration.
