Position Overview
We are seeking a dedicated and skilled Clinical Care Nurse to join our team, focusing on delivering exceptional care to patients in need of comprehensive discharge planning. This role involves both collaborative teamwork and independent responsibilities to ensure the well-being of patients during their hospital stay.
Key Responsibilities
As a Clinical Care Nurse, you will be responsible for:
- Providing high-quality nursing care to patients preparing for discharge.
- Administering personal care, medications, and intravenous treatments.
- Supporting the multidisciplinary team in facilitating patient well-being sessions.
- Participating in board rounds to ensure safe and efficient patient discharges.
- Assisting the ward manager in maintaining optimal patient flow.
