We invite enthusiastic nurses to become part of our interdisciplinary team at the Community Intermediate Care Centre (CICC), a pioneering initiative aimed at providing high-quality, integrated care for local adults with sub-acute health care requirements.
Key Responsibilities
As a member of the CICC, you will be proficient in assessing, planning, and evaluating nursing care. Your role will involve:
- Collaborating within a multidisciplinary team to deliver nursing and rehabilitation services for in-patient beds.
- Taking charge of the unit in the absence of senior staff, managing patient admissions, providing care, and planning discharges while ensuring the unit operates effectively.
- Partnering with various professionals and agencies to address the health and rehabilitation needs of patients.
- Demonstrating professional skills in care planning, medication management, and treatment delivery.
- Utilizing excellent communication skills to assist patients in understanding their conditions and promoting self-management.
- Being adaptable to a flexible shift pattern, covering 24 hours a day, 7 days a week as part of an internal rotation.
- Supervising less experienced staff and students, contributing to quality assurance and service development.
Qualifications and Experience
Essential:
- Registered General Nurse with a degree-level education (or equivalent).
- Current NMC registration.
- Evidence of recent professional development.
- Knowledge of a wide range of long-term conditions, including frailty and palliative care.
- Understanding of national health and social care initiatives and their local implications.
Desirable:
- Experience in a rehabilitation unit or intermediate care setting.
- Community nursing experience.
Skills and Attributes
Essential:
- Experience in a rehabilitation or intermediate care environment.
- Strong interpersonal and communication skills.
- Effective organizational abilities.
- Proficiency in articulating the evidence supporting patient care plans.
- Computer literacy.
Desirable:
- Experience in managing personnel and complex situations.

- Ability to work shifts over a 24-hour period as part of an internal rotation.
