Salary: £20,316 to £23,316
Hours: 37.5 hours (Standard working hours are Monday to Friday, with occasional evening and weekend availability required to meet the needs of service users)
Benefits: 30 days of Annual Leave (increasing to 35 days with length of service) + Bank Holidays, Company Pension, Life Assurance x2 & Employee Discount Scheme
Base Location: Mansfield, operating on a hybrid model of 2 days per week in the office (Note: This is a community role, requiring travel to various locations)
About Us
Polaris Community is a reputable provider of children's services, dedicated to delivering innovative and outcome-focused support, aimed at creating brighter futures for children, young people, and families.
Our team provides intensive assistance to children and young people transitioning from residential care to fostering placements, stabilizing existing placements, supporting reunification efforts, and preventing children from entering care.
Requirements
This is a Community Role, and travel to various locations is essential.
- Experience in working with children and families in home visiting roles or delivering parenting programs.
- Ability to collaborate within a professional network (including foster carers, social workers, and educational institutions) to advocate for the needs and preferences of families.
- Willingness to engage in ongoing learning and development to enhance your role as a Family Support Practitioner and deliver evidence-based therapeutic programs.
- Residence in the surrounding areas to facilitate community visits.
- A full driving license with unrestricted access to your own vehicle.
Responsibilities
- Assist families who have engaged with the Cohort C Reunification and Cohort D Edge of Care programs.
- Deliver evidence-based therapeutic support to parents and caregivers within the community to promote placement stability.
- Listen and respond empathetically to children, young people, and their caregivers to foster positive changes.
- Maintain regular contact with children or young people and/or their caregivers through face-to-face and telephone interactions.
- Ensure that services provided uphold principles of equality, diversity, and individual rights.
- Implement services according to the established operational guidelines.
- Collaborate with other professionals and agencies to achieve the best outcomes for children, young people, and their families, ensuring a cohesive approach.
