Job Title: Community Care Assistant

Job Summary:

The Care Bureau is seeking a skilled and compassionate Community Care Assistant to join our team. As a Community Care Assistant, you will play a vital role in providing high-quality care and support to individuals in their own homes.

Key Responsibilities:

  • Deliver personalized care and support to clients in their own homes, promoting independence and dignity.
  • Assess client needs and develop personalized care plans to meet their unique requirements.
  • Provide assistance with daily living activities, such as bathing, dressing, and medication management.
  • Support clients with rehabilitation and reablement goals, promoting recovery and independence.
  • Collaborate with healthcare professionals and other stakeholders to ensure seamless care coordination.
  • Maintain accurate records and reports, adhering to regulatory requirements and company policies.

Requirements:

  • Valid driving license and access to a vehicle.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Compassionate and empathetic approach to care.
  • Ability to work flexible hours, including evenings and weekends.
