Advanced Nurse Practitioner

About the Role
We are seeking an experienced and motivated Advanced Nurse Practitioner to join our team at Southern Health NHS Foundation Trust. As an Advanced Nurse Practitioner, you will be responsible for providing high-quality, patient-centered care to our patients, working collaboratively with clinicians, nursing teams, and other members of the multidisciplinary team.
Main Responsibilities
- Assess, diagnose, and treat patients with acute medical conditions, using a medical model to assess and prescribe.
- Use a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team.
- Participate in and influence efforts across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management, and assure early intervention.
- Provide strong leadership to the team, including day-to-day leadership responsibilities, development, and creative leadership, responsive leadership, enabling the team to achieve a 'well-led' culture.

- Relevant nursing/health degree

- Clinical Supervision Training
- Independent and supplementary Nurse prescribing course
- Teaching and Assessing qualification ENB 998 or equivalent/mentorship qualification PETALS
