We are seeking a highly skilled and dedicated Adult Mental Health Practitioner to join our team at Lincolnshire Partnership NHS Foundation Trust.
About the RoleThis is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality care to individuals with complex mental health needs. As an Adult Mental Health Practitioner, you will be responsible for conducting assessments, delivering therapeutic interventions, and coordinating care plans to ensure the best possible outcomes for our patients.
Key Responsibilities- Conduct comprehensive assessments and develop individualized care plans to address the unique needs of each patient.
- Deliver evidence-based therapeutic interventions, including cognitive-behavioral therapy, psychodynamic therapy, and medication management.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
- Provide education and support to patients and their families to promote independence and resilience.
- Participate in regular team meetings and contribute to the development of care pathways and service improvements.
To be successful in this role, you will need:
- A degree in mental health nursing or a related field.
- Registration with the Nursing and Midwifery Council (NMC).
- Experience working with adults with complex mental health needs.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and as part of a team.
