Job Overview

An exciting opportunity has arisen for a dedicated and compassionate individual to join our Stroke Specific Early Supported Discharge Team in the East BCUHB area.

Key Responsibilities
  • Gathering of baseline information to assist Therapy Assessments.
  • Carrying out therapeutic interventions following initial assessments carried out by the Physiotherapist and Occupational Therapist.
  • Interventions will include progression of transfers and mobility, delivery of exercise programmes including exercise classes and practicing activities of daily living such as washing and dressing and meal preparation.
  • Working independently to promote wellbeing, safety and independence within essential activities of daily living, by providing instruction, advice and treatment to the service-user and/or their carers.
About the Role

This role is an excellent opportunity to work as part of a multidisciplinary team, providing specialist stroke rehabilitation tailored to each patient's needs, working towards the goals that are important to them, to optimise their quality of life after leaving hospital.

The post holder will be responsible for carrying out therapeutic interventions delegated by the qualified therapists, in the patient's place of residence.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
