About This Role
We are seeking a highly motivated and experienced Adult Congenital Heart Disease (ACHD) Specialist Nurse Practitioner to join our dedicated team at the Oxford University Hospitals Heart Centre. This is an exciting opportunity to make a real difference in the lives of patients with ACHD, providing expert care and support throughout their journey.
Your Responsibilities
- Provide comprehensive clinical assessment, diagnosis, and management of patients with ACHD.
- Develop and implement individualized patient care plans in collaboration with multidisciplinary teams.
- Educate patients and their families about ACHD, treatment options, and self-management strategies.
- Liaise with other healthcare professionals to ensure seamless transitions of care.
- Contribute to the development and implementation of clinical guidelines and protocols for ACHD care.
- Participate in service evaluation and quality improvement initiatives.
About You
You will be a registered nurse with advanced training and experience in cardiology, specifically in the field of ACHD. You possess excellent clinical skills, communication abilities, and a passion for providing patient-centered care.
Essential Requirements:
- Cardiothoracic nursing experience
- Senior Band 5 or Band 6 level of practice
- Postgraduate cardiothoracic course
Desirable Requirements:
- Master's degree in Nursing (MSc)
