Main Responsibilities
- The incumbent will oversee the daily operations of the Wound Care Service, ensuring the delivery of high-quality clinical care within various healthcare settings. They will offer professional guidance, support, and education to multidisciplinary teams, patients, and caregivers, adhering to best practice guidelines in wound management.
- The candidate will be knowledgeable and skilled, continuously enhancing their expertise in response to patient and service needs.
- As an autonomous practitioner, the individual will collaborate with system partners and contribute to the Primary Care Networks and Integrated Neighbourhood Teams as part of the out-of-hospital care model.
- A valid UK driving license and access to a vehicle are essential for this role.

Role Responsibilities
Clinical and Professional Duties
- Conduct comprehensive assessments of pressure ulcer risk and wound conditions, identifying factors that may compromise tissue health.
- Design, implement, and evaluate care plans for patients referred to the service in collaboration with referrers.
- Promote health and well-being, empowering patients to make informed decisions and encouraging independence where appropriate.
- Ensure accurate and detailed documentation related to tissue viability.
- Foster positive working relationships within multidisciplinary teams, providing specialist advice and support.
- Adhere to NMC Guidelines and Trust Policies, recognizing personal competencies and limitations.
- Serve as a role model, demonstrating professional leadership and competent practice.
Patient Care
- Perform holistic assessments of patients referred to the service, utilizing various assessment methods to reach reliable, patient-centered conclusions.
- Provide expert nursing advice for patients with tissue viability issues across multiple settings.
- Ensure all advice is evidence-based and aligns with Trust and national policies.
- Follow safeguarding protocols for vulnerable individuals.
Communication Skills
- Maintain confidentiality in patient records and adhere to policies regarding patient privacy.
- Communicate effectively, both verbally and in writing.

Qualifications and Skills

- Registered professional
- Experience in community healthcare settings
- Completed relevant diploma or degree
