Job Summary

Aintree Park Group Practice is seeking a highly skilled and dedicated Advanced Nurse Practitioner to join our dynamic team in North Liverpool Primary Care Network.

Main Responsibilities
  • Assess, diagnose, and treat specific diseases in accordance with agreed medical and nursing protocols.
  • Provide direct access clinics and telephone triage for undifferentiated patients with minor illness and ailments.
  • Undertake Chronic Disease Management clinics for patients with a diagnosis of Asthma, COPD, Diabetes, Ischaemic Heart Disease, Hypertension, and Rheumatological conditions under the supervision of General Practitioners and according to agreed medical and nursing protocols.
  • Record contemporaneous and accurate records of delivered care on the Health Information System, including appropriate SNOWMED codes.
  • Participate in the sharing of home visits amongst the wider team of clinicians.
  • Prescribe safely, effectively, and cost-efficiently within boundaries of agreed competencies.
  • Attend and participate in practice meetings as a nurse representative.
  • Initiate and lead practice developments in first-contact nursing to enhance the nursing contribution and quality of care.
  • Provide support and training for nursing and medical students attached to the practice.
  • Delegate tasks to appropriately trained clinical and administration staff.
  • Review own requests for blood results/investigations.

  • Confident and professional with excellent communication and clinical skills.
