Belmont Recruitment is seeking a skilled Community Care Officer to join a Local Authority team on a temporary basis. This position is full-time, requiring 37 hours of work per week, Monday through Friday, with the possibility of hybrid working arrangements after initial training.
The Adult Duty Team is dedicated to assisting individuals aged 18 and older who present a variety of needs. The team focuses on providing support to both adults and their caregivers through comprehensive, person-centered evaluations, employing a strengths-based methodology in their interventions.
Collaboration with partner organizations is essential, as the team strives to empower adults to sustain their safety and independence within the community for as long as feasible, emphasizing early intervention and preventive measures.
Key Responsibilities:
- Conduct Care Act assessments, either in person or via telephone, based on the assessed risk and individual needs, while considering mental capacity.
- Ensure the provision of suitable services to protect vulnerable adults and their families, tailored to their assessed requirements.
- Perform assessments regarding the needs of adults and their caregivers, adhering to departmental eligibility criteria and national performance standards within designated timeframes.
- Develop and execute culturally sensitive care plans and services that reflect flexibility and innovative resource utilization, while also taking on a care coordination role.
- Manage case responsibilities, including proactive caseload management (incorporating risk management), monitoring, coordinating, and reviewing care plans and ongoing service delivery.
- Work autonomously under the general guidance of the Team Leader, effectively managing deadlines and adapting to frequently changing circumstances and competing priorities.
- Support team members in their duties by providing guidance, assistance, and mentorship as necessary. The role may also involve accompanying service users, performing escort duties, assisting with home visits, and gathering information.
- Maintain and produce professional case records and reports that are structured, clear, accurate, timely, and evidence-based, in accordance with departmental case recording standards and practices.
Essential Qualifications:
- Proven experience in a social care environment working with adults.
- Familiarity with supporting adults facing various challenges, including learning disabilities, mental health issues, substance misuse, and physical health concerns.
- Understanding of safeguarding principles and the ability to identify individuals at risk, demonstrating flexibility, resilience, and the capacity to prioritize and adapt to changing situations.
- Competence in conducting adult-focused needs assessments within a person-centered framework.
- Experience in developing and implementing individualized care plans that showcase flexibility and innovative resource application.
- Knowledge of costing and commissioning care packages.
