About the Role

We are seeking a highly skilled and experienced Clinical Case Manager to join our team at Compass Associates. As a Clinical Case Manager, you will be responsible for managing a caseload of clients with catastrophic brain and spinal injuries, providing expert care and support to help them achieve their rehabilitation goals.

Key Responsibilities
  • Manage a caseload of clients with complex neurological conditions, including brain and spinal injuries
  • Conduct thorough assessments and develop personalized care plans to meet the unique needs of each client
  • Collaborate with multidisciplinary teams, including healthcare professionals, family members, and caregivers, to ensure comprehensive care and support
  • Provide expert guidance and support to clients and their families, empowering them to make informed decisions about their care
  • Monitor and evaluate client progress, making adjustments to care plans as needed to ensure optimal outcomes
Requirements
  • Registered with the NMC, HCPC, or SWE
  • Experience working with clients with traumatic brain, spinal, or neurodegenerative conditions
  • Strong background in neuro rehabilitation and case management
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
