We are seeking a highly skilled and compassionate Clinical Pharmacist to play a crucial role in patient care at Berkshire Healthcare NHS Foundation Trust.

Main Responsibilities:
  1. Collaborate with clinical colleagues to develop and implement the clinical pharmacy service for the UCR and Virtual Frailty Wards programme.
  2. Conduct thorough assessments of UCR and Virtual Frailty Wards prescription charts to ensure safe and effective prescribing practices, including identifying potential drug interactions and providing expert advice for clinical monitoring.
  3. Work closely with community teams and local acute trusts to ensure seamless transfer of care for patients into the community.
  4. Develop and maintain strong relationships with community teams, primary care colleagues, and GPs to ensure comprehensive pharmaceutical care throughout the patient's journey.

Person Specification:
  • Masters Degree in Pharmacy or equivalent, with successful completion of pre-registration training year.
  • Pharmacy Honours Degree (BPharm or MPharm) or Postgraduate Clinical Pharmacy Diploma.
  • Member of the General Pharmaceutical Council (GPhC).
Requirements:
  • Substantial post-registration experience in Hospital and/or Community Pharmacy.
  • UK driving licence and car ownership, as regular travel across the 3 locality sites (Wokingham, Reading, and Newbury) is required.
