Job Title: Flexible Practice Nurse

Job Summary:

The Flexible Practice Nurse will play a vital role in providing high-quality nursing care to patients at Southview Medical Practice. This is an exciting opportunity for a skilled and dedicated nurse to join our team and contribute to the delivery of excellent patient care.

Key Responsibilities:

  • Assist GPs in the implementation of chronic disease management plans, ensuring effective care and treatment for patients.
  • Contribute to the development of policies, protocols, and guidelines within the practice, promoting evidence-based practice and quality care.
  • Aid the practice team in the implementation of new initiatives for chronic disease management, working collaboratively to improve patient outcomes.
  • Support the practice nursing team in the provision of evidence-based guidelines and care, promoting best practice and quality care.
  • Promote the development of integrated services within Primary Care, including integrated practice-based teams and multidisciplinary working.
  • Provide professional and managerial support to healthcare assistants, reviewing and monitoring patients within primary care.
  • Offer guidance and support to healthcare assistants to improve standards of care, as needed.
  • Participate in chronic disease management clinics, providing practical assistance and professional advice to achieve minimum standards of care.
  • Encourage nursing practice in line with relevant evidence-based research, promoting best practice and quality care.
  • Act as a resource to the general public, patients, and healthcare professionals, providing information and support as needed.
  • Work alongside healthcare assistants to develop their skills and knowledge, supporting their professional development.
  • Support the practice in achieving quality indicators and targets of the GMS contract and relevant National Service Frameworks.
  • Perform basic nursing duties, including venepuncture, ECG, wound care, and management.
  • Undertake male, female, and family planning assessments, providing advice and referrals as needed.
  • Perform cervical screening and promote breast awareness, ensuring high-quality care and patient outcomes.
  • Undertake accredited training in specific chronic disease management, as required.
  • Undertake first aid and management of emergencies, ensuring patient safety and well-being.
  • Contribute to the provision of equitable, high-quality services for patients in the locality, promoting best practice and quality care.
  • Help support the integration of primary, secondary, and tertiary care services, ensuring seamless patient care.
  • Maintain an interest in national research initiatives, disseminating information to healthcare professionals as needed.
  • Ensure all care is evidence-based, using up-to-date validated literature.
  • Provide professional advice and support to healthcare professionals within the Primary healthcare team.
  • Participate in training and facilitation of other practice nurses and healthcare assistants, promoting professional development and best practice.
  • Provide mentorship and guidance for healthcare assistants, supporting their professional development.
  • Participate in partnership programs of awareness raising, detection, and prevention of chronic diseases, promoting best practice and quality care.
  • Maintain a level of expertise in specific chronic disease management and other pertinent areas of healthcare, seeking to develop new and improved skills.
  • Advise members of the public on health promotion, education, prevention, and protection, promoting best practice and quality care.
  • Seek out new knowledge of nursing and healthcare by reading, enquiring, and participating in continuous education programs.
  • Take responsibility for personal development and education, including in-service training.
  • Direct, supervise, and help mentor nurses and healthcare assistants in the clinical setting, promoting best practice and quality care.
  • Actively participate in multidisciplinary education and training, promoting best practice and quality care.
  • Maintain professional development by attending relevant courses and study days, ensuring up-to-date knowledge and skills.
  • Abide by legal requirements and statutory rules relating to practice, maintaining periodic professional registration with the RCN and NMC.
  • Participate in an annual appraisal, ensuring ongoing professional development and best practice.
  • Maintain communication and liaison with other disciplines across primary, secondary, and tertiary care services, as needed.
  • Contribute to and work effectively within the primary care team, promoting best practice and quality care.

Requirements:

  • Registered Health Professional with a valid registration with the RCN and NMC.
  • Experience in primary care nursing, with a focus on chronic disease management.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, healthcare professionals, and other stakeholders.
  • Ability to work independently and as part of a team, with a focus on delivering high-quality patient care.
  • Flexibility and adaptability, with a willingness to learn and develop new skills.
  • Ability to maintain confidentiality and adhere to practice policies and procedures.
