We are seeking a highly skilled and experienced Advanced Nurse Practitioner to join our team at Tredegar Health Centre. Main Responsibilities- Diagnose and manage patients with acute and chronic conditions, including those with undiagnosed or undifferentiated diagnoses.
- Prescribe and dispense medications as necessary, and refer patients to secondary care or organize emergency admissions when required.
- Conduct face-to-face, telephone, and video consultations with patients, and make home visits as needed.
- Manage a caseload of patients, ensuring timely and effective care and treatment.
- Collaborate with other healthcare professionals, including GPs, nurses, and clinical pharmacists, to provide comprehensive care and support.
- Develop and implement care plans, and monitor patient progress and outcomes.
- Participate in quality improvement initiatives and contribute to the development of clinical policies and procedures.

Requirements- Registered Nurse with a Master's degree or Postgraduate Diploma in Advanced Practice, or equivalent qualification.
- Qualified Independent Nurse Prescriber, with experience in prescribing and managing medications.
- Minor Illness Qualification, with experience in managing acute and chronic conditions.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong clinical skills, with the ability to diagnose, manage, and treat patients with a range of health needs.
- Ability to work autonomously, with minimal supervision, and to make decisions in a fast-paced and dynamic environment.
