Job Opportunity

Corporé, a leading provider of rehabilitation case management services, is seeking an experienced Associate Case Manager to join their team.

The ideal candidate will have a background in Occupational Therapy, Nursing, or Physiotherapy and be able to work UK-wide.

As a key member of the team, you will carry out immediate needs assessments and proactively case manage clients who have experienced life-changing injuries.

Using your excellent communication skills, you will support your clients in identifying and achieving their full potential, providing goal-focused rehabilitation to facilitate an optimum return to work and function.

Key Responsibilities:

  • Conduct immediate needs assessments and develop personalized case management plans
  • Provide goal-focused rehabilitation to support clients in achieving their full potential
  • Collaborate with clients, families, and healthcare professionals to ensure comprehensive care
  • Monitor progress and adjust plans as needed to ensure optimal outcomes
