Key Responsibilities

Reporting to the ICU Ward Manager, you will collaborate with the team to provide personal care to patients with various rehabilitation and care requirements.

As part of a passionate and dedicated healthcare team, you will be responsible for delivering exceptional care to patients in our community wards. This role is open to professionals who possess the right qualities to enhance our team. The position involves rotational shifts across a 24-hour period.

  • Deliver high-quality, patient-centered care that aligns with the principles of the "Essence of Care" and "Standards for Better Health".
  • Facilitate the Expected Date of Discharge (EDD) through collaboration with patients, caregivers, the team, and internal/external agencies to ensure a safe and effective discharge that meets the patient's needs.
  • Communicate effectively with patients and their families/caregivers.
  • Ensure the implementation of effective patient-centered care programs through appropriate leadership.
  • Monitor quality and compliance with standards on an ongoing basis.
  • Maintain a safe and healthy environment for patients, visitors, and staff.

Job Duties

  • Conduct physical health assessments, including venepuncture and other necessary tasks.
  • Provide support across inpatient units as required.
  • Assist the nurse in charge in maintaining essential clinical supplies in inpatient units.
  • Enhance the service user experience by actively listening, addressing concerns, and resolving issues collaboratively with colleagues and senior team members.
  • Maintain professionalism in interactions with service users, caregivers, colleagues, and other agencies at all times.
  • Collaborate with other Trust services and external agencies to ensure seamless care for service users.
  • Observe and report any changes that may indicate alterations in the general wellbeing and safety of a service user, including their presentation, behavior, mood, and verbal communication.
  • Provide personal care to service users as needed, demonstrating empathy, dignity, respect, and compassion.
  • Ensure that all entries made in nursing notes are current, relevant, and accurate, with content reported and countersigned by a registered nurse or relevant professional.
  • Work in partnership with service users to access a variety of activities and services that promote their independence and healthy lifestyle choices.
  • Ensure the ward environment is clean and orderly at all times, coordinating with the ward housekeeper regarding cleanliness.
  • Support registered staff in addressing the physical health, comfort, and wellbeing of service users, including taking vital signs under their guidance and supervision.

Qualifications and Experience

Essential:

  • Registered Nurse
  • NMC Registration

Desirable:

  • Postgraduate qualification related to ward specialization
  • Mentor qualification / ENB 998 or equivalent
  • Additional qualification for specialization

Skills and Abilities

  • Demonstrated commitment to high-quality care and service delivery
  • Effective verbal and written communication skills, including IT literacy (MS Office)
