Position Overview

Key Responsibilities

Clinical Duties:

Effectively manage undiagnosed and complex patient presentations through comprehensive data collection, assessment for critical indicators, diagnosis, treatment planning, and follow-up within the defined scope of practice.

Candidates must exhibit advanced clinical reasoning skills, employing problem-solving techniques to evaluate and integrate information, addressing medical uncertainties across diverse contexts and clinical scenarios, including physical, mental, and social health issues.

Possess a thorough understanding of referral processes, including routine, urgent, and emergency admissions.

Utilize resources efficiently and judiciously.

Manage both acute and chronic health conditions with a focus on medical complexity.

Effectively conduct consultations through various modalities, including telehealth, video conferencing, and in-person visits.

Prescribe medications and treatments in accordance with local and national guidelines, where permitted, as an independent prescriber.

Provide personalized care and connect patients with other healthcare professionals or community resources as necessary.

Maintain accurate, timely, and comprehensive records of patient interactions, adhering to relevant legislation and organizational policies.

Support the on-call triage system within the practice.
