We are seeking a highly skilled and compassionate Primary Care Nurse to join our team at Practice Plus Group. As a Primary Care Nurse, you will play a vital role in delivering high-quality patient care and contributing to our nursing strategy and vision.
Key Responsibilities- Provide outstanding evidence-based care to patients, seeking out, listening to, and acting on their feedback to ensure personalized and informed care.
- Deliver non-judgmental care that makes a real difference, as defined throughout our bespoke Health in Justice competency framework.
- Contribute to our nursing strategy and vision by providing a nurse-led service, preventing hospital admissions, and undertaking medication rounds, dispensing controlled medication, attending emergency response situations, assessing patients, and undertaking reception screening.
- Use your skills and knowledge to deliver nurse-led clinics to your patient group, while being supported by a multi-disciplinary team.
- Registered nurse Adult
- Current professional qualification (NMC)
- Evidence of Continued Professional Development
- A non-judgmental and compassionate approach
- Understanding of clinical governance, health and safety, QOF, EBP, and national standards
