Job Summary:

The Discharge Coordinator will play a crucial role in ensuring the timely, effective, and safe discharge of patients from the community hospital setting to the community. This involves coordinating relevant services, liaising with social and health service colleagues, and attending weekly Multi-Disciplinary Team (MDT) meetings and ward handovers.

Key Responsibilities:

  • Work in close liaison with the ward team to facilitate the timely, effective, and appropriate discharge of patients from the community hospital setting to the community.
  • Develop and implement an innovative service providing excellent discharge practices within the ward environment.
  • Ensure best practice in planning safe and timely patient discharges to achieve best outcomes for patients and optimize community hospital bed utilization and reduce length of stay.
  • Take responsibility for ensuring all necessary arrangements are put in place to optimize discharge, including issuing and acting upon the protocol of choice letter to patients/families.
  • Act as the focal point of contact for up-to-date information in relation to discharge communication, particularly for patients and carers, providing good quality up-to-date relevant written discharge information.

Requirements:

  • Assess, plan, implement, and evaluate programmes of care with minimal supervision.
  • Work as a member of the Multi-Disciplinary Team without direct supervision, supporting senior nursing staff in promoting health, independence, and maximizing the quality of life of patients, clients, and carers.
  • Promote a high standard of evidenced-based nursing care and dignity for patients at all times.
  • Assist senior nurses with ensuring patients' nutritional status is maintained and maximized to facilitate patient recovery and well-being.
  • Assist senior nurses with ensuring a clean ward environment for patient care and best practice in infection control.
  • Support the Ward Matron and Sister/Charge Nurse to ensure performance indicators are achieved, e.g., infection rates, average length of stay, delayed discharges.
  • Participate in audits as required and support the Ward Matron and Sister/Charge Nurse in the implementation of action plans.
  • Participate in, and take responsibility for, keeping within the agreed departmental budget under the management and guidance of the Ward Manager and Sister/Charge Nurse.
