About the Role

We are seeking a highly skilled and experienced Senior Nurse to join our Integrated Discharge Service (IDS) as an Advanced Nurse Practitioner Service Lead. This is an exciting opportunity to lead a team of nursing staff and ensure that we provide a patient-focused discharge service to patients across our sites.

Key Responsibilities
  • Lead the discharge planning process and provide expert clinical and professional leadership to nursing staff at all levels within the IDS Team.
  • Ensure that nursing care is delivered to a high standard and that the nursing resource is managed effectively.
  • Work closely with staff across all disciplines between hospital and community to ensure safe, timely, and effective transfer of patient care on discharge.
  • Provide senior support to the Admission Avoidance Service in the Emergency Department and Acute Medical Unit.
  • Manage the Patient Lounge and hold a case load of patients with highly complex discharge requirements.

Requirements
  • First level registered nurse.
  • Leadership or Management Qualification.
  • Substantial experience of teaching and mentoring more junior staff.
  • Leadership of complex discharge management in acute hospital setting.
