What will your responsibilities include?
- Providing tailored, one-on-one coaching and support to a designated group of tenants
- Implementing a comprehensive, strengths-based coaching program tailored to each tenant's needs
- Identifying personal objectives for tenants and maintaining focus on their progress
- Creating and updating individualized support plans that outline goals and achievements
- Conducting and revising risk assessments for each tenant
- Advocating for tenants as necessary
- Encouraging tenant participation in additional services and activities to foster personal development
- Offering emotional support to tenants
- Challenging negative lifestyle choices and promoting accountability
- Preparing tenants for transitions to independent living when appropriate
- Empowering tenants to enhance their independence and wellbeing
- Assisting tenants in developing healthy habits, such as engaging in physical activities and social events
- Supporting tenants in accessing mental health resources
- Maintaining regular communication with tenants in accordance with organizational standards
- Keeping accurate records of interactions, risk assessments, and support plans
- Reporting any safeguarding concerns to the appropriate authorities as required
- Collaborating with statutory and voluntary organizations that support tenants
- Staying current with required training for the role
- Completing any additional tasks as needed
